I agree with full understanding that Melissa Lucciola, CHt (the hypnotherapist) is not a medical doctor, nor a licensed mental health practitioner and does not diagnose or treat any physical or mental disorders. I hereby waive and release any and all claims of personal injury that may arise from the hypnosis sessions and/or recordings. I further agree that the hypnotherapist assumes no responsibility for the outcome of the process and for guaranteeing its efficacy. I certify I am a competent adult assuming these risks and take full responsibility for the results. This waiver and assumption of risk is effective as of today and may not be revoked, altered, amended, rescinded or voided, without prior written consent of the hypnotherapist.
